You may request a copy of a death certificate only if the person passed away in Israel.

You may only request a copy of a death certificate for someone to whom you are related in the first degree (i.e. spouse, parent, sibling or child) or if you have a court order requesting it.  You must provide proof of your relation to the deceased or the relevant court order.

You can make the request either in person at the consulate or by the mail. It takes between 2-4 weeks for the certificate to arrive at the consulate, at which point it is sent back to the requester.

In order to request a copy of a death certificate at the Consulate, you will need to provide following:

  • A valid photo ID.
  • $9 payable by cash, credit card or money order made payable to "The Consulate General of Israel in New York."  We do not accept personal checks.

Request a copy of a death certificate by mail

If you cannot come to the consulate in person you can request a death certificate by mail.  To do so, you will need to send us the following:

  • A completed icon Request for a Death Certificate (1.21 MB). This must be signed before a notary public.
  • An exact mailing address of where you would like the certificate to be sent and your phone number.
  • $9 payable by cash, credit card or money order made payable to "The Consulate General of Israel in New York."  If you choose to pay by credit card, please include a completed icon Credit Card Payment Form (1.74 MB)
